* You must provide an original or paper copy of all required documents at your appointment.
Registrations CAN NOT be finalized until all requirements & steps (documents, registration forms, and closeout meeting) have been completed.
Please allow for 1 hour to review your documents, complete the online registration forms and meet with our enrollment specialists.
* We will scan in your documents and hand them back to you.
