Required Documents
Parent/Guardian MUST complete the registration process by
in-person appointment at the Enrollment Office.
Registrations CAN NOT be finalized until all requirements/steps (documents, registration forms, and closeout meeting) have been completed.

Proof of Residency
We need a total of 3 items:
- One of the following: lease, new home contract or mortgage statement, tax affidavit, section 8 agreement with current date/signature
- Plus two additional items: i.e. utility bill, bank statement, or pay stub, etc. (documentation must be within 60 days of the current date)

Student Documentation
- Original Birth Certificate for the child
- Custody/Guardianship documents filed with the courts
- Immunization Records current and complete
- Photo ID of parent/guardian enrolling the child
- Grade Verification: students in grades 1- 9 need current report card / students in grades 10 - 12 need unofficial transcript
- Students entering kindergarten do not need grade verification
- IEP and ETR or 504 with signature page if applicable

NEW STUDENT REGISTRATION
During your enrollment appointment time, please allow up to 1 hour to review documents, complete the online registration form and have a closeout meeting.

STEP 3
Closeout Meeting
|
The Closeout Meeting happens immediately following the completion of STEPS 1 & 2
This is an individual meeting where we will review your enrollment forms and you will
learn about your child’s school:
- Uniform Policy
- Building specific information
- School supply lists
- Grade-level information
- Answers to your questions!
The enrollment office is open for appointments Monday - Friday from 8 a.m. - 3 p.m.
